Human Oversight Must Include the Power to Change the Outcome

A reviewer who cannot inspect, reject, or stop an AI recommendation is not a meaningful control.

Anthropic's October policy announcement reiterates qualified human oversight for specified high-risk uses, including recommendations affecting health, legal rights, finances, livelihood, or essential services. The AI Act framework also describes human oversight among high-risk system obligations. Neither should be reduced to adding an approval button.

Match the reviewer to the consequence

A qualified reviewer needs the domain knowledge to recognize an error and the authority to change the recommendation. Asking a general administrator to rubber-stamp a specialist decision may create the appearance of oversight without its substance.

Give reviewers access to the relevant source material, uncertainty, and policy context. Do not require them to approve a conclusion while hiding the evidence needed to challenge it.

Design for real workload

Review queues can fail when volume, deadlines, or incentives make rejection impractical. Measure queue age, reversal rate, review time, and escalations. Investigate unusually uniform approvals rather than treating them automatically as proof of quality.

For irreversible or high-consequence actions, require review before execution. A post-hoc audit may help detect a problem but cannot undo every harm.

Test the control

Use safe simulated cases with missing evidence, contradictory sources, and clearly unsuitable recommendations. Confirm that staff can stop the action and route the case to someone with the right authority.

Document the roles and procedures appropriate to your actual legal and contractual obligations. Meaningful oversight is an operating capability: the organization can recognize a problem, intervene in time, and explain the decision that followed.
