AI Incident Response Must Track Actions as Well as Answers

A response plan for agents that can change records, contact customers, or trigger downstream workflows.

A mistaken AI answer and an unauthorized AI action need different recovery steps. As platforms connect agents to business tools, incident plans must account for changed records, sent messages, and downstream automations, not just problematic text.

Define the incident boundary

Identify events that require escalation: sensitive-data disclosure, unauthorized writes, repeated incorrect recommendations, unexpected external communication, and loss of control over an ongoing task. Set thresholds based on consequence rather than treating every typo as a major incident.

Make stopping safe

Assign someone who can disable the agent, revoke tool access, and pause scheduled work. Test whether stopping the visible conversation also stops background actions. A chatbot can disappear from a page while a queue continues executing tasks.

Preserve the right evidence

Record the model or agent version, relevant inputs, retrieved sources, tool attempts, authorization decisions, and resulting changes. Protect that evidence with appropriate access and retention controls; an incident log can itself contain sensitive information.

Distinguish attempted actions from completed actions. Recovery should be based on what the underlying systems actually changed, not solely the agent's narrative.

Recover before expanding

Use approved correction procedures, notify the appropriate stakeholders, and assess reporting obligations with qualified advisers. Then update evaluations and controls to cover the failure.

NIST's lifecycle risk-management approach supports this continuous feedback loop. The aim is not a claim that incidents will never occur. It is an organization that can detect them, limit their impact, and make a justified decision about returning the system to service.
